Discussion for Week 1

This class uses a different approach to discussions.Each student is assigned to one of four groups (Group 1-4) and each group has 2 weeks during the 8 week class in which they are responsible for the initial response postings, and the other 6 weeks the groups respond to others' postings. The responses will be done by each member of the group; this is not a "group project," but just a way to divide the class. To determine which group you are in, go to My Tools > Groups.

Topic: Describe the roles of technology, people, information, and organizational components in health care information systems.

The readings for this week discuss health care organizations and systems, including systems to support administration, billing and finance. For your Stage 1 assignment, you will describe the roles of people, technology, processes, organizational components, and data/information in the EHR system for the Midtown Family Clinic. For this discussion, you will practice explaining these roles as they relate to a billing system for a hospital.

GROUP 1:As regards a billing system for a hospital, describe the role of each of the following:

1. People – who are the people that use the billing system? What other people are affected by the billing system?

2. Organizational Components – what departments use the billing system? What other departments or outside organizations might be involved?

3. Processes – what activities (processes) are the users of the system engaged in? (please do not just say "billing"!)

4. Data/information – list 5 data elements or information items that are important in the billing system.

5. Technology – how does technology support the billing system? What are some expectations you have about how the technology performs?

GROUPS 2,3 and 4:In replying to the postings, you are to critically evaluate all of the following for at least two main postings:

1. Are the explanations of the roles of each of the five components listed above clear, complete and accurate? If not, explain.

2. Is there a clear relationship between each of the five components described and the hospital billing system?

Topic: Analyze organizational processes to identify information systems requirements

For your Stage 1 assignment, you will analyze the processes at the Midtown Family Clinic to identify the essential requirements for the EHR system for the Clinic. For this discussion, you will practice analyzing processes and identifying functional requirements for a system to improve a process.

First, we need to be sure you can identify a process; many students have difficulty with that, so refer to the week 2 readings that describe various healthcare processes. Keep in mind that a process is a set of specified steps to accomplish a task.

Second, requirements for a system need to be clearly written so that the people who are developing the system or evaluating a system for use can discern whether the requirements are met or not. One set of criteria for writing clear requirements is referred to as "SMART" criteria, which means each requirement statement is:

o Specific – Clearly and simply stated, and not likely to be mis-interpreted. Avoid such words as: and, or, but, soon, immediately, somewhat.

o Measurable - Can be verified as complete or not. Avoid such words as: best, fastest, optimally.

o Attainable – Able to be achieved; appropriate.

o Realistic – Considering the project and other requirements, can this one be expected to be met.

o Time-bound – If appropriate to the requirement, specifies when or how fast a requirement needs to be completed.

GROUP 2:

1. Drawing from your own experience, select a health care process used at your place of work or in your interaction with a health care organization that you would like to see improved. Explain why you picked that process.

2. Imagine that a system is to be implemented (or an existing system improved) to make that process better, and identify five (5) requirements that the system would need to satisfy. Each requirement is one sentence in length and addresses one thing the system must do. Here we are interested in "functional requirements" – the activities the system must perform to support the identified process. We are not including "nonfunctional requirements" that include how the system would perform.

3. Review (and re-write if necessary) the 5 requirements statements to ensure they meet the "SMART" criteria.

GROUPS 1, 3 and 4:In replying to the postings, you are to critically evaluate all of the following for at least two main postings:

1. Should the process identified actually be considered a "process"; that is, does it meet the definition of "a set of specified steps to accomplish a task"?

2. Do the requirements listed support the selected process?

3. Are the requirements clearly stated such that system testers will be able to ascertain whether or not the requirement has been implemented (have they followed the "SMART" criteria)?

4. What improvements to the requirements would you suggest? At least one is required; explain what needs to be corrected and provide your re-wording of the requirement.

Topic: Describe the data/information that flows between health care organizations.

The Week 3 readings focus on data – data quality, data storage, and data records (EHRs). For your Stage 2 assignment, you will identify external organizations with which the Midtown Family Clinic needs to share data, identify what data elements will be shared, and whether the data is going to the external organization or is coming from the external organization. In this discussion, we will examine what interoperability (data sharing) is, why it is important, and why it is so difficult. First, you should conduct some research and find out what interoperability is and how it impacts or is used in health care systems. GROUP 3: Using a rehabilitation facility as the organization,

1. List three data elements (or items of information) that the rehab facility would need to receive with a patient referral [Review what others have posted and try to come up with some different data elements]

2. List one unique (not previously listed by another respondent) data element that the rehab facility would need to provide to an external organization, and identify what type of organization that is and the purpose of the data sharing.

3. Choose one aspect of interoperability that you found in your research and explain what an EHR system for the rehab facility would need to be able to do to exchange information with other organizations.

1. Are the data elements listed appropriate to the information that a rehab facility would receive with a patient referral? If not, explain.

2. Topic: Selecting an EHR system - understanding cloud implementations.

As part of the Stage 4 assignment you will propose an appropriate EHR system for the Midtown Family Clinic. In addition to whether or not it is a certified EHR system, one of the most important aspects of the system is whether it will be implemented on-site at the Clinic or will be hosted "in the cloud." This discussion will help you understand what a "cloud-based system" is and what benefits and challenges come with that implementation choice.

GROUP 2: Conduct some research on cloud-based systems or, specifically cloud-based EHR systems, and:

1. In your own words, explain what a "cloud-based" system is and how it differs from a system implemented on the premise of the using organization

2. Identify two benefits of using cloud-based systems

3. Identify two risks or drawbacks of using cloud-based systems

Topic: Discuss the considerations for implementing the enterprise system solution in the organization.

Since it is very likely that you have not had the opportunity to research and select a system, or to address the considerations for implementing a system, this discussion is designed to allow you to practice the things you will need to do for your Stage 4 assignment.
